Death occurs in Hospital

1.    Engage a Funeral Director (click here for list of Funeral Director)

a.    The Funeral Director will walk you through the whole procedure from downloading of the Digital Death Certificate (E-DC) to the procedures on the day of the funeral.
b.    The Funeral Director will collect the body, send for embalming (if required), and deliver the body to the location of the Wake.  

2.    Obtain the Digital Death Certificate Number from the doctor after certifying the cause of death.

(Note: The doctor may refer the case to Police/Coroner if cause of death is unnatural. In such cases, the body will be sent to the Centre for Forensic Medicine (CFM) Mortuary at Blk 9 of Singapore General Hospital)

3.    Download Digital Death Certificate (E-DC)

a. Download the Digital Death Certificate from My Legacy using this QR code or weblink .

b. Download the digital death certificate as soon as possible. Once downloaded, the digital death certificate can be stored in the next of kin’s personal devices.

c. Information required for the issuance of the digital death certificate

i. Digital Death Certificate Number (to be obtained from doctor)

ii. Deceased’s NRIC Number

iii. Deceased’s date of death
